Enrollment Requirements

 

School Hours:  8:00 a.m. to 4:00 p.m. (Call school for hours of Registration)*
*Many schools require appointments. Please contact the school to schedule an appointment.

Materials Needed for Registration:

Enrollment Packet– Please obtain an Enrollment Packet from any HSD School Site or from the District Office. Have this completely filled out prior to starting enrollment at the school site.

Please bring:

1) Two (2) Proofs of Residency
Mandatory:  Title Papers, Mortgage Payment, or original Rental Agreement with name and phone number of owner/manager/management company on it.

Secondary: Utility Bill: Gas, Electricity, Telephone/Cell phone, or TV Cable.  Must be current within two months & no Past Due bills can be accepted unless paid receipt is provided.  

Secondary: Car, Homeowner, Rental Insurance, Car Registration or other as approved by school personnel.

If the above mentioned proofs are in someone else’s name, that person must accompany the parent to the school with identification and name, address and phone number of the apartment owner or manager so as to confirm that the parent and the student reside at that address, and must also sign a Declaration of Residency form. A home check will be done within 30 days after placement of the student.

2) Parent Identification: Photo I.D. Card or Driver’s License of parent or Guardian

3) Proof of Age

For Kindergarten Students, one of the following:

  • Birth Certificate
  • Affidavit of Birth
  • Baptismal Certificate

For 1st through 8th Grade, one of the following:

  • Transfer or Report Card from Previous School with birth date listed
  • Birth Certificate, Affidavit of Birth or Baptismal Certificate

4) Proof of Grade/School Attendance

  • Transfer papers
  • Last Report Card given
  • If student is in any Special Education program(RSP, SDC, and Speech) parent must provide a current IEP. An appointment will be set up for placement with the school psychologist.

5) Immunization Records

For Kindergarten:

  • Including Mantoux TB Test within 1 year of enrollment
  • Including Varicella immunization

For first time enrollment in a California school:

  • Must include recent Mantoux TB Test

For Grades 1st thru 8th:

  • Up to date record of immunization

6) Physical Examination Record for 1st Graders

  • Must have doctor's signature and stamp

7) Oral Health Assessment (Dental Screening) for new enrolling Kindergartners or 1st graders

  • To be completed during the first year of enrollment by May 31st.

8) Assessments for Placement: Parent must bring student for language fluency testing if another language other than English is spoken in the home.
